The error is in the pass:
"9) Test a rewritten URL, such as -www.yoursite.com/phpBB/post1.html, if you end up with a 404 (not a phpBB message), go back to step 8;"

My server returning error 404, and redirecting to index.

Could you tell us a bit more about your installation, like where is phpBB installed (dir) and where did you install the .htaccess ?
Is the forum online so I can check it ?

About the cache/, did you properly chmoded the phpbb_seo/cache/ dir ?

The redirecting to index after the 404 must come from your server settings, is there an .htaccess somewhere already ?
