I am wondering if someone might be able to confirm if I am correct or not...

I do not want to have my forum name and description listed within my forum's header so I have simply left it blank within the ACP >> Board Settings page. Now that I am using the phpBB SEO pre-mod (3.5) version, I would have to hard code that into my functions/include.php file in order to have that information available to search engines?

You'd better remove the code displaying your site title and description form your overall_header.html template, because site name and description are used elsewhere too where it can be useful, just like in the meta tag mod. But you can as well hard code the value in the meta tag mod's code as you suggested.
Next version of the meta tag mod will make these setting configurable from ACP (when using the Ultimate SEO URL mod)
