So assuming that this .htaccess is actually placed in www.dilmiljaye.com/ try to replace :

or :

RewriteRule ^/forum\.html$ index.php [QSA,L,NC]

if each time, you still get a 404 on -http://www.dilmiljaye.com/forum.html then you'll need to make sure that your host allow the use of mod_rewrite.
