New message Posting template

I think it would be great to have an option to have customizable templates for posting new message in the forum.

Like if I have a forum for introduction and I want people to enter their new message in a particular format like
Name
Location
Website
Introduction etc.

I shud be able to customize the posting template to output these pre-written fields so that people can just add text infront of this.

Is there any such MOD available ?

I don't know of any, but I am sure this would require quite some changes in phpBB.
