I'm using Simple SEO 0.4.8 - and without any changes...in the last week a lot of the pages show up "Page Cannot Be Found".

I can't replicate the issue although it has happened to me once. I'm getting a lot of members on the site with the same issue and I don't even know where to start on troubleshooting the issue.

mm, hard to tell. I assume that you where using rewritten urls at that time.

When I try -http://www.duckhuntingchat.com/forum/topic119800.html, I get a custom "this page has moved page" with a 404 header.

This could mean two thing, mod_rewrite is dead (either because the .htaccess was changed/deleted or mod_rewrite is just no more available), or the custom error page implementation is causing trouble.

If there where actually no change in the online files, then, it must be the server to have changed in some ways.
In practice, you can try to go through all the process of implementing the .htaccess as you first did, should help out to understand better and even could fix the issue.
