You've said you have ~40 mods installed, therefore it's likely very hard to identify the exact problem / conflict.

You can activate the following options:

Profiles and groups injection: Yes
Profiles ID removing: Yes

Keep in mind that search engine bots / spiders usually don't have access to user profiles. In this case it's more an aesthetic feature than SEO.

Do you use the following settings?

ACP -> phpBB SEO -> phpBB SEO Class settings:

URL rewriting type: Advanced (Changing this option requires an .htaccess update.)
Activate The No duplicate: Yes
Activate the Zero duplicate: Yes (Post Redirections: guest)
