Did you try purging the cache?

Try deleting the .htaccess file via FTP and create a new one via the phpSEO tab and transfer it to the appropriate directory (either your root or the forum folder) via FTP again.

Also, try deleting your browser cache as well.

Update, I started fresh, and have the mod working (kind of) and am not getting 404 errors any more. However, there are a few strange quirks that I'm getting but because I'm now using the Advanced mode, I'm going to start a new topic in the Advanced forum. See you there guys!
