It is working out of the box, so it's either a mod you installed or something wrong with the mod install (straight mistake or outdated code).

What is the error exactly (with full debug on) ?

So, you mean that you have lost attachment after you did a db backup ? Backuping the db is unrelated to the event IMO, there must have been an error (while optimizing (if you repair before you optimize it can occur) table for example) during the backup process or somewhere else. I can't really tell ...

The basic custom question (are you a bot) configured in acp, ban on all gmail.com (sadly, but it was alone providing with over 90% of spammers without any reaction from google, and user usually have an alternate email together with their gmail) and some moderation of course.

Attachments are not stored in the db, you need to make sure you also put back online the content of the phpBB/file directory (and while you're at it, the content of phpBB/donwload, phpbb/store and phpBB/images/avatars).

FYI, by "how well" I truly meant "how badly", and the canonical tag is, even though stronger than robots.txt, a weaker measure than the zero duplicate which 301 redirects all kinds of dupes to the canonical url.
